Attention Deficit/Hyperactivity Disorder Guideline

STEP 1: Present to our office documentation from your MD and psychologist or other appropriately credentialed psychoeducational professional (see the 2 parts involved below). Documentation can be mailed, faxed, or handcarried. If you mail or fax, please call to make sure it was received. Sometimes things do not make it here.

Guidelines for documentation

USC follows Educational Testing Service (ETS) guidelines


  • Must be 3 years old or less. Documentation older than 3 years is not acceptable.

  • Must be typed, on letterhead.

  • Must include 2 parts.


1. Letter from MD with qualifications listed


  • Diagnostic interview (should include any evidence of early impairment, evidence of current impairment, developmental history, family history of ADHD, relevant medical/medication history, description of current educational limitations).

  • Ruling out of alternative diagnosis (mood, neurological, other disorders).

  • Any prescribed medications for ADD/ADHD and the specific symptoms they help to control or manage for the student (i.e., inattention, hyperactivity, etc.).

  • Specific DSM-IV-TR diagnosis and code.

  • Detailed description of how this impairment significantly limits a major life activity in an academic setting.

  • Suitable ratings scales that might be included in the report are: Wender Utah Rating Scale, BrownAttention-Activation Disorder Scale, Beck Anxiety Inventory, Hamilton’s Depression Rating Scale, Connor’s Parent/Teacher Rating Scales.

  • Suitable tests of attention including: Continuous Performance Test (such as Connor’s or IVA),Test of Variables of Attention (TOVA), STROOP, Trailmaking Test.

2. Psychoeducational evaluation


  • Evaluation conducted by a psychologist or other appropriately credentialed psychoeducational professional.

  • Should include any recommendations for accommodations in the classroom setting.

  • Suitable tests for the Aptitude portion include: WJ-III Tests of Cognitive Ability, WAIS-III, or Kaufman Adolescent and Adult Intelligence Test.

  • Suitable tests for the Achievement portion include: Subtests from the WIAT, WJ Tests of Achievement, or Detroit Tests of Learning Aptitude-03 (or DTLA-A)

Step 2: Go to the Disability Services website.

Print out and submit the Student Intake Form. This will be placed with your documentation.

Step 3: Once adequate documentation AND your Intake Form have been received, a committee will review your file and determine registration status. You will then be NOTIFIED BY MAIL about the next step. If the documentation is adequate, your letter will ask you to set up an intake meeting. If needed, the letter will request additional documentation.

Step 4: Arrive at your intake meeting prepared to talk with a staff member about yourself, your disability, accommodations, and responsibilities in the process.
